To enroll your child at Aspiring Heights Academy, we will need the following:
- Birth Certificate. We only need a copy of your child's birth certificate. If your child was born in Florida, you can request a copy from the Florida Department of Health. If your child was born in a different state, you can find out where to write for vital records at the Centers for Disease Control and Prevention's website.
- School Entry Health Examination. The Florida Department of Health issues DH Form 3040, School Entry Health Exam. If you recently moved from another state, your child's school-entry health exam may be different. Any certificate will be accepted as long as the exam has been performed within one year of enrollment and was completed by a health care provider licensed to perform physical examinations. Your child shall be exempted from this requirement upon written request stating objections to the examination on religious grounds.

Certification of Immunization. We require either DH Form 680, Certification of Immunization or DH Form 681, Religious Exemptions for Immunizations. These forms are available from your County Health Department and from Florida licensed physicians. Your child shall be exempted from this requirement upon objection in writing that the administration of immunizing agents conflicts with your religious tenets or practices. Enrollment Form. We require one completed Student Enrollment Form per child. If your browser's privacy settings do not allow you to open the document, right click on the link and select Save Link As. . . to save the file to your computer.
- Annual Fee. We charge an annual enrollment fee of $20 per child for basic enrollment (Grades 1–12) and an enrollment fee of $60 per high school grade level per child for our Graduation Plan (Grades 9–12). Students in Grades 9–12 may be enrolled under our basic enrollment plan or our graduation plan. We accept Paypal and checks and money orders payable to Aspiring Heights Academy. If you pay with Paypal, be sure to include your child's name when you make your payment. If you are enrolling more than one child under the same plan, you may pay the enrollment fees in a single payment.
Basic enrollment includes support, record keeping services, free homeschool IDs, and FLVS access. Our Graduation Plan includes basic enrollment plus dual enrollment opportunities, a diploma, an official transcript, and assistance with Bright Futures scholarship applications.

If you are enrolling your child in our Graduation Plan, we also require that you provide us with your child's high school transcripts (if any) and your child's course plan for his or her current grade. Please note that we do not need official transcripts for enrollment purposes, but we will need sealed, official transcripts before we can approve dual enrollment, issue a diploma, or enter your child's transcript into the Bright Futures Scholarship database.
A student who wishes to earn a Florida Bright Futures Scholarship should not be enrolled in our school for grades 11 and 12 unless the student plans to earn a diploma through our school or plans to earn a GED and is able to document his or her GPA and coursework through official transcripts. Please read Bright Futures Scholarship Eligibility for more information.
Please mail the required documentation to the address above. Due to the nature of its contents, we recommend (but do not require) you send your documents via Certified Mail or with Delivery Confirmation. Be sure to keep a copy of everything you send for your records. You will receive a Welcome email with your child's enrollment date upon enrollment. Attendance records and future enrollment renewals may be mailed or emailed.
