To enroll your child at Aspiring Heights Academy, we will need the following:
- Birth Certificate. We only need a copy of your child's birth certificate. If your child was born in Florida, you can request a copy from the Florida Department of Health. If your child was born in a different state, you can find out where to write for vital records at the Centers for Disease Control and Prevention's website.
- School Entry Health Examination. The Florida Department of Health issues DH Form 3040, School Entry Health Exam. If you recently moved from another state, your child's school-entry health exam may be different. Any certificate will be accepted as long as the exam has been performed within one year of enrollment and was completed by a health care provider licensed to perform physical examinations. Your child shall be exempted from this requirement upon written request stating objections to the examination on religious grounds.

Certification of Immunization. We require either DH Form 680, Certification of Immunization or DH Form 681, Religious Exemptions for Immunizations. These forms are available from your County Health Department and from Florida licensed physicians. Your child shall be exempted from this requirement upon objection in writing that the administration of immunizing agents conflicts with your religious tenets or practices. Enrollment Form. We require one completed Student Enrollment Form per child. If your browser's privacy settings do not allow you to open the document, right click on the link and select Save Link As. . . to save the file to your computer.
- Annual Fee. We charge an annual enrollment fee of $20 per child for basic enrollment (Grades 1–12) and an enrollment fee of $60 per high school grade level per child for our Graduation Plan (Grades 9–12). Students in Grades 9–12 may be enrolled under our basic enrollment plan or our graduation plan. We accept Paypal and checks and money orders payable to Aspiring Heights Academy. If you pay with Paypal, be sure to include your child's name when you make your payment. If you are enrolling more than one child under the same plan, you may pay the enrollment fees in a single payment.

Please mail the required documentation to the address above. Due to the nature of its contents, we recommend (but do not require) you send your documents via Certified Mail or with Delivery Confirmation. Be sure to keep a copy of everything you send for your records. You will receive a Welcome email with your child's enrollment date upon enrollment. Attendance records and future enrollment renewals may be mailed or emailed.
